Mentor Consent & Release

This Consent & Release ("Release") is between LifeChoices LLC, a Florida limited liability company located at 4522 W Village Dr, Unit 1160, Tampa, FL 33624 ("LifeChoices") and the undersigned individual ("Mentor," "you," or "your"). It governs your participation as a Mentor whose responses and characteristics will be used to build a personal advisor profile (a "Profile") on the LifeChoices platform.

1. What you're agreeing to

You have been invited by a LifeChoices user (the "Inviting User") to complete a questionnaire about your values, experiences, and approach to decisions. We will use your answers to build an AI-generated Profile that the Inviting User — and, if you permit it below, other users — can consult when thinking through their own decisions.

The Profile is an interpretation of your answers, generated by AI. It is not you, will not always reflect what you would actually say, and may be wrong, incomplete, or contradict views you currently hold or will come to hold in the future.

2. What we ask you to confirm

By accepting this Release in the LifeChoices Mentor flow, you confirm that:

• (a) You are providing your questionnaire answers voluntarily, and you understand they will be used to build an AI-generated interpretation of your perspective.
• (b) You understand the resulting Profile may generate responses to dilemmas and questions you were never actually asked, by extrapolating from your answers and any other material you've authorized us to use (see Section 3).
• (c) You understand LifeChoices will label your Profile as an AI interpretation, and will not present Profile outputs as your verified, confirmed, or literal statements.
• (d) You understand you may revoke this consent at any time (see Section 5), but that you cannot retroactively undo Profile interactions that already occurred while your consent was active.
• (e) You are at least 18 years old (or the age of legal majority where you live).

3. What material we're allowed to use

You authorize us to use, in addition to your direct questionnaire answers:

• Any additional written material, recording, or transcript you voluntarily provide (such as a letter, essay, or audio/video file) and confirm you have the right to share.

We will not use any material to build your Profile beyond what you provide or explicitly authorize.

4. Who can use your Profile

By default your Profile is private: only the Inviting User may consult it. You may, in writing to lifechoicesai@gmail.com, widen this to specific named people, or (rarely) to anyone on LifeChoices. If you widen access, you understand strangers may ask your Profile about dilemmas you will never know about, and that your Profile will be labeled with your name and presented as an interpretation of your perspective.

5. Your right to change your mind

You may revoke this consent and request deletion of your Profile at any time by contacting lifechoicesai@gmail.com. Once we receive your request:

• We will deactivate your Profile within 5 business days, so it is no longer available to any user;
• We will delete your underlying questionnaire answers and any additional material you provided within 30 days, except to the extent we are required to retain records of the original consent and its revocation for legal or compliance purposes;
• Revocation does not retroactively affect or create liability for Profile interactions that occurred before deactivation.

6. What you're releasing

In consideration of being invited to participate as a Mentor, you release LifeChoices, its founders, employees, and affiliates from claims arising out of:

• AI-generated Profile outputs that do not accurately reflect your actual views, so long as those outputs are based on the material you authorized under Section 3 and are presented as an interpretation consistent with this Release and LifeChoices' Terms of Use;
• Use of your Profile by authorized users (per Section 4) within the scope of this Release.

This release does not cover, and you do not release any claim arising from: LifeChoices' use of material beyond what you authorized in Section 3; sharing your Profile beyond what you authorized in Section 4; or LifeChoices' breach of its own Terms of Use or this Release.

7. No payment; no employment relationship

Unless a separate written agreement says otherwise, you are not being paid for your participation, and nothing in this Release creates an employment, agency, or partnership relationship between you and LifeChoices.

8. Governing law

This Release is governed by the laws of the State of Florida, without regard to conflict-of-laws principles.

Acceptance

By clicking "I agree" in the LifeChoices Mentor onboarding flow (or by completing the questionnaire after being shown a link to this Release), you confirm you have read and understood this Release and agree to its terms.
